Unable to select checkbox or radio in popup form

Hi, I’m hoping someone can help me. For some reason the checkbox and radio buttons we have on a particular popup form are not selectable on either mobile or desktop, even though they are selectable on the form that appears on the page itself (same form).

You can view the form here, but to view the form on the popup, you must scroll down the page until the green “refer-a-friend” button appears in the top right corner of the nav bar:
