Creating a form with fields that users enter information

Hello everyone –

I’d like to create a form with “open fields,” meaning a blank field in which users type an answer. I would like those answers to then become the labels for additional “open fields.” Apologies if I’m not using proper terms, as I’m just learning Gravity Forms and the add-ons.

Here’s an example of what I mean.

Question 1: List 3 ways you help customers in your current job.
Open Field 1 – user enters “checkout”
Open Field 2 – user enters “responding to emails”
Open Field 3 – user enters “replenishing stock”

Question 2: What percentage of time do you spend on each of the follow customer tasks?
Checkout – user enters “60%”
Responding to emails – user enters “25%”
Replenishing stock – user enters “15%”

For the sake of this example, I don’t need to validate the answers or ensure they total 100%.

My questions

  1. In Gravity forms, what field type do I choose to pose Question 1 itself?
  2. In Gravity Forms, what field type do I choose for what I’ve described above as “Open Field 1,” “Open Field 2,” and “Open Field 3”?
  3. What field type do I choose to pose Question 2?
  4. What field type do I choose for the user responses in Question 1 (“Checkout,” “Responding to emails,” and “Replenishing stock”?
  5. Is there specific documentation I can reference, as I don’t see this use case in any of the tutorials I’ve found so far.

If this is not the right tool for the type of questionnaire I’m creating, I’m okay hearing that. If necessary, I believe I can use Survey Monkey or something similar to create this type of questionnaire, but I’d prefer to have everything integrated in my Website if I can.

There’s a third-party add-on that could help you to populate form fields using values from other fields: Gravity Forms Populate Anything - Enable Dynamic Population - Gravity Wiz

But Gravity Forms doesn’t provide any field type to allow users to type into field choices. I’m not aware of any third-party solution for it either. So you may need to create your own field type or hire someone to do it for you. I purchased Gravity Forms Populate Anything because I was under the impression it would work for my use case. It’s disappointing that I cannot do this with your tools, but I understand it’s an unusual use case.

Fortunately, Survey Monkey is a simpler alternative to custom development. The questionnaire just will not be as tightly integrated into my site as I’d like.

I never thought forms and surveys were so different until now. I consider myself schooled.
