Gravity form suddenly not showing up on landing page

I have a gravity form suddenly not showing up on a landing page that was previously without problems.

Recently we tested using the “shortcode” method of display in an attempt to improve pageload times.

Was working great until checked today. Switched back to the old method using the “form” widget in the client’s pagebuilder (Wordpress), but puzzlingly, it’s still not showing up.

Other sites are doing fine. Fully updated. Not sure what to check. Would rather not uninstall and reinstall the plugin, but obviously will if that’s necessary.

Anyone have this issue or have ideas? No one added any “display:none” code or anything silly like that.

Saw few errors on console might be the reason for the same.

Uncaught SyntaxError: Identifier ‘rocket_pairs’ has already been declared (at contact-us-and-request-an-estimate/:189:39)
