Hi again @chrishajer. Just when this issue was fixed I encountered another issue when I tested the form.
This form has the correct setup with its expiration date but when the client name is entered, the expiration date field gone missing.
What could have been wrong?

It sounds like there is conditional logic set on that field but it’s set up incorrectly.
