Active Campaign Add-On "Map Fields" inputs are gone (since GF 2.9.5)

When I try to create an Active Campaign feed, it no longer shows the fields to map the form to the Active Campaign fields:

It should show something like this: Creating a Feed for the ActiveCampaign Add-On - Gravity Forms

I’m running the latest versions:

• WordPress 6.7.2
• Gravity Forms 2.9.5
• Gravity Forms ActiveCampaign add-on: 2.1.0
• I’ve tried in PHP 8.0, 8.2 and 8.3

EDIT: The fields reappear when I revert to Gravity Forms 2.9.4

Have you tried clearing your browser cache?

If you have no conflict mode enabled on the Forms > Settings page, try disabling it.
