Ranking of answer for survey question

I have a question where we are looking to have the recipients rank the answers on a 1- 4 scale.

How can I get it so they can only select each number 1-4 one time for the choices?

Couldn’t you set up 4 radio fields and have the answers be 1, 2, 3, and 4?
