Phone number format not working on 2nd form, if have 2 same forms on a page

When we are having the same form multiple times on a page, the phone format (###) ###-#### on 2nd form is not working. Does anyone else face the same issue?

Note: I have already tried using the input mast as a phone field, but that does not help either.
