Gravity Forms not showing on page after excluding page from caching

Our credit card authorization form keeps getting this error “Credit Card - A profile already exists.” whenever clients try to make a payment.

We were asked to exclude the form pages from cache which we did and now the form is not visible on the page.

How do we resolve this?

PS: I have updated Gravity Forms to the latest version.
