Gravity Form connection breaking after one successful pass

We are using the Gravity / Hubspot add on and we are able to get a successful contact transfer one time but on the next submission we are getting an error (HubSpot Feed 1: There was an error when creating the contact in HubSpot. The request is not valid)

is this a known issue and how do we fix the breaking?
