Have had a form that has worked for a long time on my site,

That is a staging url I cant figure out for the life of me why it is happening, notice how the form doesn’t work initially and then it loads.

Try updating to the latest Gravity Forms interim release v2.5.16.3. It’s available here:
https://www.gravityforms.com/my-account/downloads/
