Empty Entries | All Spam Filters are Active

Permission to post, Admin.

Last 11/24/2022, we received an entry with empty fields. We can’t figure out how this happened for all the spam filters and gatchas are active. I dogged in and all I can see is an IP address without scam/spam record. I’m hoping you can help me rectify this problem for we don’t want the same occurrence in the future.

I checked this form and I found one post that is very similar to what we have: Weird empty entries, possible 500 error
