Duplication in Entries

Hello - I just set up a form to accept membership signups and donations from members of our nonprofit choral group. The form and integration with PayPal Standard all work fine. The one thing that isn’t good is that submissions appear to be duplicated when you review entries for the form (within WordPress admin). I have checked what was processed by PayPal, and know that only one payment was submitted, but it is being duplicated in our database, so that each entry appears twice. Any idea on why this would happen?

Follow-up: After first day of the form being in production, the duplication of entries in the database stopped. Strange. Don’t know if the first couple of people did something strange during form completion or what.
