Duplicate Form Entries/Notifications

Hi guys,

About 24 hours ago we started to received multiple form entries and email notifications per form submission, sometimes up to five entries per person.

Does anyone know what could be causing this? We use Zapier to transfer the data over to Hubspot, and we have several plugins in Wordpress that I’ve only recently started to update. Could the plugin updates be causing the issue?
