Create User Add-on not working (with Learndash)

I’ve had the following set up working for years and it just suddenly broke, and I’m looking for help to fix it.

  1. The user creates an account using Gravity Forms here https://learn.48in48.org/get-started/
  2. An account is created and the user is enrolled in several Learn Dash courses to be completed.
  3. The user can log in immediately and go through those courses.

However, this stopped working within the last few days. Now, this is what happens:

  1. The user fills out the registration form
  2. A user is created in WP, but it is missing the email address
  3. The user is not enrolled in any Learn Dash courses

To troubleshoot this I have:

  1. Udpated all plugins
  2. Ensured my GF license is good
  3. Checked my settings on the back end to make sure the email address is mapped correctly
  4. Checked my settings on the back end to make sure the form is set to enroll the user in the correct courses.

Please help, what else can I do to fix this issue?
