Completing fillable PDF then having it emailed

We have an insurance certificate that our customers need to send to their customers to show proof of insurance. These forms need to have the end customers’ name and address added to the document, current date, then emailed to the end customer.

The goal here is to have the customer login, enter the information themselves, and have the email come from our email address.

For bonus points, if we can have a drop down menu of the most common customers information that would be incredible.

ok similar my priject but we shoud calculate deposits too(mathic formola) and we also have authentication
