Website connecting to Mailchimp on some forms but not others

There are several forms on a website that I’m working on for a client. The forms on the Home page are connecting to MailChimp, but the same newsletter signup form on another page isn’t working.

On the home page for Adam Young Counseling both forms (First Chapter Free and Newsletter Signup) are working and submissions are going through to MailChimp.

On this page - Adam Young Counseling - Fort Collins, CO - the Newsletter Signup form is not connecting to MailChimp. This is the same form that is on the home page and working.

On this page - Free Resources - Adam Young - the form at the top of the page is not working.

All forms have the same settings and are going to the same list in MailChimp. I’m really stumped on this one.
Appreciate any help I can get.

Hello. For this issue, we will need to see the Mailchimp logs after the form is submitted. Please enable Gravity Forms logging:
