Yes. You can use the {referer} merge tag as default value for a hidden field in the form to capture that URL. We have two methods documented here:

Yes, you can use conditional logic to hide the submit button (for example) unless the email domain matches. Or you can perform custom validation after the form is submitted (before the entry is created) and return a validation error if the domain does not match.
