Possible to use single form across different sites in Multisite?

A client would like to have a multisite where they can use the same form on all sites, but with a single form maintained in one place. All entries should be stored against that single form and exported together, no matter which site the form submission was from.

Is there a way to achieve that (suggestions for custom coding welcome).
