Need help with conditional logic based on age calculation

I’ve created a form but need help with custom coding. Here is a link to the form page - https://ima.cpwebserver.com/patient-fee-agreement/. Here’s what needs to happen -

When the user enters their birthdate if they are between 18 and 49 the conditional logic will display a checkbox for $300/year = Adult Individual (between age 18 and 49) OR $450/year = Adult Individual (age 50 and over).

THEN once all required fields are filled in and they click Submit the form data is sent to Google Sheets (I can assist with the connection to Google) and they are redirected to PayPal to pay the fee based on their age.
