Multi-step form is not working with Jornaya Script

We have a multi-step form on Real Innovation Coverage Estimator - Test Form - Real Innovation

Case One:
The shortcode is [gravityform id=“11” title=“true” ajax=“true”] so with the ajax=“true” the form does not proceed after the step 2 and shows “Real Innovation Coverage Estimator - Test Form - Real Innovation” and blank screen

Case Two
When we use the shortcode [gravityform id=“11” title=“true” ajax=“false”] or [gravityform id=“11” title=“true”] then the fourth step is skipped and it shows a blank screen.

Have you tested for plugin & theme conflicts?

If you have ruled that out, do you have any conditional logic that may be interfering around step 4?

You also didn’t mention anything about a Jornaya script in your post. Can you say more about what that script is for?
