Looking for Intake Form solution

I’m a current user of Gravity Form, Gravity Perk, and a few others. I’m now looking to see if there’s a solution I can use as an Intake Form. This will be mostly, if not entirely for internal use only. I run many events and have developed a pdf form I send for various people to add details to. It’s cumbersome and can lead to errors when versioning is not tracked and waiting for someone to add their details before it can be sent to someone else to add their details.

I don’t fully understand what Gravity Flow does but it sound kinda like what I’m looking for. I also read the Spotlight on Joel Armsden’s FileStack. This sounds very much what I’m looking for.

My basic needs are to see a list of form entries, update information in it, upload files which are associated with that specific form, I would love to track when a change is made to the form. Each form entry is actually tied to a specific date so if there was a calendar view, that would be awesome! A front facing view/edit for logged in users would be handy for members of the team to add the details their responsible for.

I would love for someone to point me in the right direction. Maybe share my pdf (it’s rough) so my thoughts/needs on the process can be better envisioned.
