Hi, One of the forms I am creating requires the user to affirm everything on a list like below–how do I make it so that one cannot proceed unless all three are checked?

I affirm that –
:white_circle: I am 21 years old
:white_circle: I have read the disclaimers
:white_circle: I will submit the required paperwork

Hi Yumei,

To ensure that all three options have been selected before submitting the form, you can enable Conditional Logic for the Submit button and make sure that all three options are selected before submitting the form, as shown in the screenshot below.

Now the submit button will only be visible when someone selects all three options.
