I have a “question”, and I would like to collect multiple inputs, i.e.
Who in you family has diabetes ?
Myself
Spouse
Child
Sibling
Parent
etc, etc,
Any bright ideas on how to capture and display there ?
There are a whole list of questions like this re: family health history.
