I would like to group checkboxes from a single field with group headers, similar to the solution proposed here:

The form in that screenshot is using three different checkboxes fields, one per each disease, and using the gf_list_inline CSS ready class to put the choices inline.
Having a single checkboxes field using groups is not a feature of Gravity Forms.
