Gravity forms Chained Selects Not showing on frontend

Hello
I have a form which was working fine earlier with Gravity Forms and Gravity Form Chained Selects but suddenly it has stopped working.
If i create chained select, it is showing perfectly fine on backend but it is not showing any drop downs on frontend. On frontend, I can only see field label.

I have also checked HTML with inspect element to confirm that if Selects are hidden due to any CSS or JS, and that is not the case either:

In HTML, there is label only.

Can someone, confirm, where i am doing wrong.

I’m not able to replicate the issue in a default WordPress installation. Make sure to run latest version of the Chained Selects add-on (1.5) and Gravity Forms (2.5.16). If you have these versions already installed, I would recommend you to perform a full conflict test.

Due to the nature of the issue, in this case you need to perform the conflict test manually following all the steps described here: Testing for a Theme/Plugin Conflict - Gravity Forms Documentation

Thank you for your response.
If further tested and here are my findings.
1- Create two versions of any site (eg: Staging or Production) or take two sites A & B.
2- Install Similar plugins and themes on both sites.
3- On Site A, create a gravity form with chained select. It will work fine.
4- On Site B, Create another gravity form with chained select. It will also work fine till this step.
5- Now make some changes on gravity form on Site A and then Export that specific form from Site A.
6- Import Exported form in Site B (form exported from Site A in step 5).
7- Chained Select will stop working on Site B on all forms. It will not work on new forms either.

In our case, we have staging, dev and production sites and with same plugins and themes. Right now chained select is broken on Dev site, after exporting form from Staging and importing to Dev site.
