Form not being registered even though customer submits it

it has recently come to my attention that some of the forms being submitted by users aren’t being registered on our site. People will call saying they’ve filled out a certain form, but the form has not data for them or any submission connected to them. I have no way of even telling how many submissions I’ve missed, and no idea how to fix the problem, as there’s no obvious evidence from the site that there’s anything wrong (other times form submissions come in just fine), apart from the many people who have been contacting me that they submitted a form that I’m not seeing.
