Dynamic Membership Site for Therapist Site

I want to combine gravity forms with Wordpress (potentially a membership plugin) with GF charts to create a client profile that allows me to track changes in their personal functioning over the course of treatment. A website that does something similar is Login | Your Personality

Any ideas on how I may be able to create this?
