Conditional Notification JS Error

Hi guys, am on the latest version 2.5.15.1

When I edit a notification I can tick the bottom conditional box but no options appear below it.

If I then save the form and refresh the box is back unticked.

Here are some JS console errors I can see:

Uncaught TypeError: gform.initializeOnLoaded is not a function
Uncaught TypeError: gform.initializeOnLoaded is not a function
Uncaught TypeError: gform.applyFilters is not a function

Can anyone tip me in the right direction?
