Conditional Logic on Notification on Checkboxes won't Save

Hello,

I have checkboxes on my form and want to send a notification only when certain checkboxes are checked.

I change is to what I want and then when I save, it changes to one specific checkbox (not the one I want).
It happens to be the last checkbox.

Any thoughts?
