Pagination unexpected error on multi page form

Well the problem basically is:

I have a form with 7 pages, and when on first page the user awnsers, and when he clicks next, an unexpected error occurs and is sent to the last page.

Issues when debuging:
When i insert pagination my check box field(it’s only one of this type) doesn’t displays all of my 6 options only 5 of them, saying one is hidden, on form builder in wordpress

NOTE:
THERE ARE NO PLUG-IN CONFLICT!(as it is the awnser for everything …)
