Name field label misaligned

Hey!

I’m trying to figure out why the name field label (and only the name field label) in my Gravity Form is misaligned and what I can do (in terms of CSS, I guess?) to get it back in its place. Is it possible to give me pointers without diving into the theme CSS? It shows up fine in the preview.

Additionally, for some reason, two checkboxes show up when I insert the checkbox field.

I unfortunately can’t submit a ticket because I inherited this site and the Gravity Forms license from a media company that ran it previously, and I don’t have access to the account with the license.
