Looking for a portal style form add on

I am looking for an add on for gravity that allows a form completion with:

  • entry fields for the user to fill out (duh!)
  • Uploading documents (that we can see in entries screen)
  • Individual entries view: Users can re-login and see their uploaded documents and upload new versions
  • Conditional logic with the ability to limit uploads based upon time and date. (multiple)
  • An admin field so we can see and comment on the user :slight_smile: visible only to us.

Does anyone know of an Add On or plugin that does this?

Sounds like a combination of Gravity View, limit submissions by gravity wiz, and regular admin fields.
