Checkbox / Radio buttons not aligning properly [RESOLVED]

I can’t figure out how to align my checkboxes and radio buttons with the text. Right now, they are all dropping down lower. Could anyone assist with this? Here is a link to the form:

Thanks Chris for taking a look. My apologies, I linked to the main page and the form is on another page. But, I actually just figured out how to fix the issue with some CSS. Thanks again!
