Cant work out why my form fails to submit

My form on this page

fails to send with all fields populated.

Forms on other pages seem to submit without issue…

I have debugged to the best of my ability but cant work out what is causing the issue.
Any help on understanding what might be causing this would be greatly appreciated.

Were you able to resolve the issue with your form? I did perform a test and it appears to be working. After submission a was shown the confirmation message
