Hi,

I want to have a single form appear on multiple pages/posts.

Is there a way for Gravity Forms to automatically capture the page/post where data was submitted?

It automatically does. Check the Merge Tags in your Notification for example:

Is there a way to send that info as a form field to mailchimp?

Add a single line text field to the form, then use one of the merge tags as “Default Value”. That will store the information in a field, and you can map that field on your MailChimp feed.
