Hi Rory. It will be difficult for us to troubleshoot from here. Please open a support ticket [https://www.gravityforms.com/open-support-ticket/technical/](https://www.gravityforms.com/open-support-ticket/technical/) and include the following:

1. URL where we can see the form
2. Form export ([instructions](https://docs.gravityforms.com/exporting-a-form-from-gravity-forms/))
3. System status report ([instructions](https://docs.gravityforms.com/checking-environment-details/#h-sending-system-reports))

Thank you.
